Step 1: Print the Applicant Fingerprint Form (FD-1164)  or (FD-258)

First, you’ll need to print the “Applicant Fingerprint Form” (Form FD-1164). You can use either cardstock or regular printing paper to print out the fingerprint template. Step 2: Get Your Fingerprints Taken

If you are unsure where to go, your lawyer or facilitator can suggest fingerprinting locations. Asking fellow ex-pats or local police officers may also help direct you to suitable locations.

You will need one fingerprint card, which is sufficient, but you can submit up to five fingerprint cards per request to ensure the prints are clear and readable.

 

Step 3: Complete & Sign the Identity History Summary Request Form (OMB-1110-0052)

Next, fill out the Identity History Summary Request Form. If you’re using Elite Documents to handle your processing, our mailing address is pre-filled for your convenience. You can update the address section accordingly to send the documents directly elsewhere.

 
Step 4: Mail the Signed Form(s) and Completed Fingerprints for Processing

After completing the forms and taking your fingerprints, mail everything to us for processing. Ensure you get a tracking number to monitor the package. Send your documents to:

Step 5: Wait for Processing

Once you’ve mailed your documents, they typically take 2-3 weeks to process, though this can vary depending on the FBI’s current workload. Use this waiting period to ensure all your other paperwork is in order.

 

Step 6: Receive and Review Your FBI History Summary

Upon receiving it, we will forward you a PDF copy of your FBI report. Carefully check the report for accuracy. The original hard copy can be sent anywhere to its destination.

  3. Send Us Your Fingerprints:

    • Mail us the original fingerprint cards and the completed form using a tracking service (DHL, EMS, UPS, FedEx)
    • You can submit up to 5 fingerprints per request.
  4. Notify Us:

    • Send us an email with your tracking number so we can monitor the arrival of your package. 
  5. We will email you a confirmation upon receiving your documents and the invoice. Using the online invoice provided, you can make payment via credit or debit card.

If your fingerprints are rejected due to quality issues, you may need to submit new fingerprint cards. You can submit up to 5 cards per transaction. If the request is still rejected, we can resubmit them for a third time. If they are still rejected after the third attempt, we can then request a certified letter of “Unsuccessful Identity History Summary Check Request” with an electronic signature, which can then be apostilled by the federal agency, the U.S. Department of State.

It is recommended to have your fingerprints taken by a professional to ensure they meet FBI standards. Self-taken fingerprints may not be accepted due to quality issues.
