How to Obtain Your FBI Identity History Summary:

A Step-by-Step Guide

Step 1: Print the Applicant Fingerprint Form (FD-1164)  or (FD-258)

First, you’ll need to print the “Applicant Fingerprint Form” (Form FD-1164). You can use either cardstock or regular paper for this. Do not sign the form until you are with the fingerprinting agent. Fill in your details accurately and keep this form handy for the next step.

 

Step 2: Get Your Fingerprints Taken

Locate a certified fingerprinting agency nearby. This could be a local police station, sheriff’s office, or a private fingerprinting service. You may also go to any participating U.S. Post Office locations nationwide. Schedule an appointment if necessary, and remember to bring a valid form of identification. If you are unsure where to go, your lawyer or facilitator can suggest fingerprinting locations.

 

Step 3: Complete & Sign the Identity History Summary Request Form (OMB-1110-0052)

Next, fill out the Identity History Summary Request Form. If you’re using Elite Documents to handle your processing, our mailing address is pre-filled for your convenience. You can update the address section accordingly to send the documents directly elsewhere.

 

Step 4: Mail the Signed Form(s) and Fingerprints for Processing

After completing the forms and taking your fingerprints, mail everything to us for processing. Ensure you get a tracking number to monitor the package. Send your documents to:

Step 5: Wait for Processing

Once you’ve mailed your documents, they typically take 2-3 weeks to process, though this can vary depending on the FBI’s current workload. Use this waiting period to ensure all your other paperwork is in order.

 

Step 6: Receive and Review Your FBI History Summary

We will forward you a copy of your FBI report upon receiving it. Carefully check the report for accuracy.

  • If the report is clear, we will process the apostille certificate.
  • If the report indicates a criminal record, we will provide a copy for further verification with your lawyer or facilitator to ensure it’s safe to proceed.
